Ordering and Delivery

There are a number of different ways you can order a compounded medication.

1. 1. Print form which you can download here
   2. Complete and sign
   3. Give to the patient, fax to pharmacy or scan and email to the pharmacy
   4. You can also email a copy of your prescription to compounding@thompsonspharmacy.com.au
